The NV4500 is a heavy-duty 5-speed manual used in GM and Dodge HD trucks. Built for severe-duty use, it's in many contractor and farm trucks with hundreds of thousands of miles. Common issues are 5th gear snap ring failure, countershaft bearing wear, and input shaft damage from clutch work.

The 5th gear snap ring fails under sustained highway use, allowing 5th gear to walk back and disengage.
Symptoms: 5th gear pops out at highway speed
High-mileage NV4500s develop countershaft bearing noise β a drone that increases with vehicle speed.

Input shaft wear from clutch drag or abuse.

The snap ring holding 5th gear retainer wears or fails. It's a known design limitation. Updated snap rings are available that fix the problem.
GL-4 only. GL-5 fluid has extreme-pressure additives that attack brass synchros. We've seen many NV4500s destroyed by GL-5 fluid.
